Endoscopic Sinus Surgery with Turbinate Reduction (ESS TURB)
Anesthesia Implications
Position: Supine, arms tucked, head slightly extended
Time: 30-60 min (short)
Blood Loss: Low (10-50 ml)
Post-op Pain: Moderate (3-6)
Maintenance Paralytic: Ask surgeon
Anesthetic Approaches
- GETT
The Anesthesia
Bleeding – the primary concern will be bleeding from the nasal turbinates. Thoroughly suction and extubate awake.
Tube positioning – The surgeon will be standing on one side or the other. Make sure the ETT and lines are positioned to the contralateral side.
Tucked Arms (general considerations): Consider a second IV – once the procedure has started, it’s going to be VERY difficult to handle IV issues – especially if your only IV has problems. Ensure the IV is running and monitors are still functioning after tucking the patient’s arms
