Perioperative Insulin Calculator

One screen for perioperative insulin: toggle between subcutaneous correction and an IV infusion, set the patient's insulin sensitivity, and slide in the blood glucose to get the dose, the next titration move, and the orders — with the clinical rationale a tap away.

The perioperative insulin calculator puts the whole decision on one screen — pick the route, dial it to your patient, and read off the dose. No stepping through separate screens to choose an approach and then hunt for a number.

Subcutaneous or infusion, your call

Toggle between SQ correction and an IV infusion, and the tool shows the right output for each — a correction dose for subcutaneous, or a starting rate and titration guidance for an infusion.

Tuned to your patient

Tap the factors that shift insulin sensitivity — age, renal function, BMI, steroids, total daily dose — and the calculator sorts the patient into sensitive, standard, or resistant and scales the dose to match. Slide in the current blood glucose and the suggestion updates live.

Start or titrate an infusion

Already running a drip? Switch to Titrate, enter the current and previous glucose, and the tool tells you whether to increase, hold, or decrease — and by how much. Starting fresh? It gives you the initial rate, with clear hold-and-treat thresholds when the glucose is low.

Every number, explained

A guidance card lays out the orders — including the dextrose-and-recheck steps for hypoglycemia — and the brain icon opens the full rationale: route choice, sensitivity tiers, intraoperative targets, and the newer agents to hold before surgery, refreshed against the 2024 SAMBA consensus.
